This pdf contains Chicago Department of Public Health’s official presentation of findings from the 2014 city-wide “Healthy Chicago” survey, which examined 7 aspects of individual health: official “health status”, health care and access, diet and physical activity, tobacco use, chronic disease, mental health, and emergency preparedness. The survey was conducted via telephone, with 2,500 non-institutionalized Chicagoans of ages 18 and older participating. The findings within are broken into the 7 examined aspects of health, with the data stratified by age, race-ethnicity, sex, and income-level.
